Full Medical Staff Application-This application is for Physicians who would like to apply for full medical staff privileges at CSH. Please return the completed application to Andrea Webb, in the HIM Department. For the application packet, please click here. Temporary Medical Staff Application-This application is for Physicians who want to receive temporary privileges for a specific patient or if the physician is in need of immediate privileging (pending Governing Board approval) of their full medical staff application. In this instance, the full medical staff application must accompany the temporary application. Please return the completed application to Andrea Webb, in the HIM Department. For the application packet, please click here. Allied Health Application-This application is for members of the Allied Health Profession (nurses, nurse practitioners, etc). This application must be completed and co-signed by the supervising physician. The supervising physician must be a member of the CSH medical staff.
